For each new service, all this data, such as tests, medicines, prescriptions, recipes and questionnaires will be attached to the Clinical record of the Patient. Therefore, it is concluded that the Clinical Chart is the patient's medical file, where they are stored all the medical records that, while the even own ties with the company.

It is very useful to medical professionals of the company, as it provides for consultation, the main contributor data, such as: medical records, tests, personal data, questionnaires answered, previous calls, recipes, exam result items, referrals, testimonials/ASO, departures and images, facilitating the monitoring of the health of the met.

This query is given stating the code chart, the number of months to consult and the selection of subjects: exams, medical records, personal data Questionnaire, Attendances, recipes, Items Result, referrals, Testimonials/ASO, Clearances, Local histories, Images, histories, Medicines and Vaccines.

By pressing the button show, will be presented with a window with the selected subjects in a tree on the left and the right information. The information that have record such as testimonials, calls, etc., can be viewed quickly by clicking on the selected date.

The window has a feature that allows you to rebuild the window as standards. To use it should be pressed the right mouse button and select the option Restore Dimensions standards (Shift + Ctrl + D).

Last Months Indicate the number of months that the program should check to get the information. The maximum allowed value for this field is 999.

Group by SSN Mark this field so that in consulting the medical records of employees, are considered the results of exams, attendance, medicines, recipes, testimonials/ASO, clearances, questionnaires, vaccines, referrals made in other contracts from the developer. The filter will not be applied to non-medical records of employees (type Serviced = 1). To mark the field, the user will know that the filter will be used when the SSN is being shown next to the field.

Period Inform the starting and ending date of the period you want to consult. Will always adjust the period reported for a period which is not greater than 999 months.

Selection of Subjects
Tick what items should be displayed on the window. The right are two arrows in opposite directions which may be used for the ordering of these subjects or click with the mouse on it and drag to the desired position.

Selection filters

• Area: Inform the area that you want to consult or leave zero to consider all.
• Situations: Report situations to consult or leave blank to see all.

PCMSO

• Yes: Tick this field to consider only the IE tests with PCMSO origin other than "Request".
• Not: Mark this field to consider only those tests that are not related to the PCMSO i.e. tests with the source type "request".
• All: Tick this field to consider all the tests generated or registered from all backgrounds.

Beyond the limits
Mark this field to consider only those tests that have opinions above acceptable limits.

Within the limits Mark this field to consider only those tests that have opinions within acceptable limits.

Below the line Mark this field to consider only tests that have opinions below the limits.

No results Mark this field to consider items that do not have results. To consider all it is necessary to leave blank all items.

Vaccines
Inform the vaccines that should be considered in the research.

Situation
It indicates the type of the desired situation and may be:

• Requested: matches the vaccines that have not been applied
• Held: matches the vaccines applied
• Cancelled: Believes that vaccines will not be applied
