Complementary Form

This item contains complementary information related to the worker, third party and partner, and they have been separated from the employee register because they were not mandatory, allowing that the user fill them later, according to its time availability.
Note
By clicking the right mouse button on this window, or clicking the Other Windows button, the system will display a menu with the options: Employee Form, Partner Form and Third Party Form. These options allow to display the Employee, Partner and Third Party admission form. By selecting one of them, the employee register for the selected registration will be open in the complementary form.
This register has mostly fields that do not need a history, but when they are changed they generate pending issues in the eSocial layout S-2205.
Key fields
Report the type of worker. Each type has a different use in the Human Capital Management | HCM. See below the characteristics of each one:
- Employee: is the type used for the employees, workers or employees registered in the company.
- Third Party - is indicated for workers from other companies who render temporary service. For this type, there is the Foreseen Period field, in which the user may type the beginning and end dates of rendering service of this third party.
- Partner - It differs from a third party because it is a worker registered in other company located in the industrial park of the company owning the Human Capital Management | HCM module. This is an intermediate type between the employee and the third party.
All types are processed in the evaluation, cafeteria and payroll integration routines; all types also are considered in the access routines.
Indicate the ID number of the worker, third party or partner. The quantity of positions and edition of the ID number must be previously defined, in Others > Indications item.
Form
Registration of necessary data so that the company can use the electronic register form.
Register Form
Demonstrates the Register Form number of electronic control. A sequence is automatically determined, when including a worker. Allows to indicate another number, for example, the number of the registration original form in the Ministry of Labor.
To carry out automatically the numbering of each Branch, the last number used must be indicated, in Companies > Branches.
Select the desired administrative region among those registered in Tables > General > Administrative Regions.
City Report the code of the address city. The system displays the cities with their respective codes according to the RAIS* codification.
ZIP CODE
Report the Zip Code of the worker, third party or partner address.
This field will be automatically filled out when reporting the neighborhood code, according to the parameters from the Tables > Cities or Tables > General > Cities window, except when this Zip Code field is positioned on the window above the Neighborhood field.
Address
Report the code of the address where the worker, third party or partner lives.
The addresses can be registered on the window of Cities, available in the Tables > Cities or Tables > General > Cities menu item, depending on the module used. Such registration is not mandatory, however, if carried out, when reporting the Zip Code for this address in the Zip Code field, the system will automatically fill out the Address, City and Neighborhood fields, except when they are already filled out, because the information typed by the user will remain. Thus, the user only needs to indicate the number and the address complement, if there is.
Note
It is not possible adopting this treatment for cities which have unique ZIP CODE.
The filling out of the Tax Identification Number is mandatory only from 2011. This information started being requested in the DIRF* generation.
Report the code of the worker, third party or partner birth city.
The system presents the cities information, according to the RAIS* codification, being necessary only to select the city.
Type of Civil Certificate
Indicate the type of civil certificate. It is in the 15th digit of the registration number.
Issuing Date of Civil Certificate
Indicate the registration date of civil certificate.
Civil Certificate Registration
Allows inserting the registration information, with until 32 digits, when the Issuing Date of Civil Registration is greater or equal to 1/1/2010. In other civil certificates with issuing date less or equal to 12/31/2009, it is possible to report only the Term, Book and Sheet fields.
Civil Certificate Term/Registration
Fill out with digits from the 24th position until the 30th position of the civil certificate registration number.
Registry of Civil Certificate Registration
Indicate the registry where the civil certificate has been registered.
Civil Certificate Book
Indicate the book where the civil certificate is. It is from the 16th to the 20th digit of the registration number.
Civil Certificate Sheet
Indicate the sheet of the book where the civil certificate is. It is from the 21st to the 23rd digit of the registration number.
Professional Board
Report the professional advice they belong to employees, third parties and partners, often necessary, as in the case of accountants, engineers, etc.
The different types of Council, can be registered in the item > Tables > General Advice.
Registration in Professional Board
Report the number of registration in the professional board.
Registration Status
This field will be used when issuing the PPP*, to compose the number of the professional board registration in the professional board.
According to the Normative Instruction, it must be filled out with D, in case of permanent registration and P in case of temporary registration.
If the worker is not responsible for the PPP* information, as environmental registrations and biological monitoring, this information is not necessary.
Note
This field is available for the Personnel Management module, if it is integrated with the Security and Medicine, Time and Cafeteria Control, Headcount and Budget, Jobs and Salaries, Legal, Trainings and Research and Profit Sharing Plan modules.
Enter the employee's registration number at the Ministry of Labor (MTb), if any.
This field is only available for modules: Personnel Administration; Recruitment and selection; Security and Medicine.
Issuing Date
Report the day, month and year of the of the professional registration issuing.
Expiration of Registration
Report the day, month and year of the expiration for the registration in the professional board. It can be used for temporary registers.
If this field is Shipping date are informed, the Registration Maturity must be a post-shipment date for the S-2200 layout generation.
Note
This field is available only if there is integration with the Headcount and Budget, Jobs and Salaries and Time and Cafeteria Control modules with the Trainings and Research module.
Contract Duration
Indicate the term of the probation contract, in number of days. Based on this information, its expiration will be calculated, allowing to issue reports about expired deadlines and deadline to overcome, as well as its extension.
Asset Clause
Please state whether the contract for a specified period contains an assertion clause of the reciprocal right of termination, before the date of its termination. This is used for the generation of eSocial layout S-2200.
For this field to be enabled, you must also have informed a duration for the contract.
Extension of Work Contract
Indicate the deadline of the probation contract, in number of days. Based on this information, its expiration will be calculated, allowing to issue reports about expired deadlines and deadline to overcome, as well the control of contract conclusion.
Last Medical Exam
Report the day, month and year of the worker, third party or partner last medical examination.
This field can be automatically used if the database is integrated with the Security and Medicine module, in Medicine > Definitions - PCMSO* folder, the Examination Complementary Form field contains the code of the desired medical examination. This field will be updated with the last register found on the Worker’s medical Examination Results window (Medicine > PCMSO* > Results or Medicine > Audiometry > Results) with the medical examination status equal to C-Conducted.
Next Medical Examination
Report the month, day, and year when the next medical examination will be conducted.
This field can be automatically used if the database is integrated with the Security and Medicine module, in Medicine > Definitions - PCMSO* folder, the Examination Complementary Form field contains the code of the desired medical examination. This field will be updated when registering a medical Examination Result (Medicine > PCMSO* > Results or Medicine > Audiometry > Results) with Request Date posterior to the system date, which medical examination status be equal to R-Requested.
Qualification
Some companies use this data to determine the qualification required for certain jobs, among other. Allowing you to identify the qualification with a code created by the company.
Locker Number
Allows to report the number of the locker that the worker uses to store its belongings.
Beginning of Use
Report the start date and end date for using the locker.
Name of the Building
Report the name of the building where the worker works. This information is useful mainly in the Senior Access Control and Security Management system.
Floor of the Building
Report the number of the building floor where the worker works. This information is useful mainly in the Senior Access Control and Security Management system.
Report the worker full name, when it exceeds the 40 positions of the Name field from the Employee Register.
Notes
- The system does not assume automatically that the name has been filled out in the Employee Register because this Full Name field is informed, the system no longer uses the information from the Employee Register field in layouts that support more than 40 positions. Example: SEFIP.
- Through the reports generator, it is possible to define a variable on the entry window of the model, allowing that the user selects if wants to print this full name or the name from the Employee Register.
Default Structure
Report the default salary structure for the worker job. It will be used as a parameter in reports about studies of salary status related to its job.
When admitting a worker in structure 1, but with possibility to reach structure 3, the structure 3 must be reported as default structure.
Default Class
Report the default class for the job, understood as ideal, which will be used in reports to analyze the salary status.
If applicable, enter the social name for transvestite or transsexual.
Health Care Plan

Data record of the health plan of the dependent, collaborator, third party or partner.
IPE*
Warning
This documentation applies only to the module: AP.
When the worker is statutory, register the items of this window for purposes of IPE* calculation.
Documents
With this option, the user can store a series of scanned documents from the collaborators. To do this, just fill in the fields:
Document Date
Report the document date.
Type of Document
Report the type of this document. The types of document are registered in Tables > General > Types of Documents.
Document Sequence
Report the code of the document sequence. It is used because the worker can have many types of documents from the same type and with the same data.
Click the Attach button to select the image file.
Notes
The use of the Rule per Entity is available in this tab, for the R034DOC table (except the After Deleting option).
Documents attached to the Contributor Document Photos table (R034DOC) can not have a name greater than 246 characters. This value still counts the file extension.
Electronic Addresses

Note
The Electronic Addresses tab is enabled only if the Personnel Management module is integrated with the Recruitment and Selection module, and in the Recruitment and Selection module in Others > Indications, the RS Version field is defined as 1 - New RS.
Type
Report the code of the type of email address that you want to register.
Notes
- The same type of electronic address can be registered more than once. In this case, a different sequence (in the Seq column) is created for each one.
- When registering a video, it must be embedded. In some websites, when right-clicking on the video, the “Get Embedded” option is displayed. On other websites, like Youtube, the option of the Share button must be used and after the Get Embedded option, and in the code that will be displayed, copy what is quoted in the source property, which will be represented by "src".
When clicking the Sare button and after the Get Embedded option, the following will appear in the text box:
"<iframe width="425" height="349" src="http://www.youtube.com/embed/glofSn7xLDw" frameborder="0" allowfullscreen></iframe>". When copying the embed in the source (src), looks like this: "http://www.youtube.com/embed/glofSn7xLDw".
Description (Type)
This field is automatically filled out according to what is reported in the Type field.
Document Sequence
Report the code of the document sequence. It is used because the worker can have many types of documents from the same type and with the same data.
Address
Report the email address.
Data Abroad
Allows registering an address for the worker who works abroad. This information is used to generate the DIRF* if the worker has proceeds while it is abroad.
Beginning Date
Report the beginning date when the worker started living abroad.
TIN (Tax Identification Number) Abroad
Tax Identification Number (TIN) provided by the tax administrative body abroad, indicator of natural person or legal entity. The filling out of the Tax Identification Number is mandatory only from 2011.
End Date
Report the end date when the worker finished living abroad.
Temporary Contract
In this guide, if applicable, inform the employee's temporary engagement data.
Inform the start date of the temporary collaborator's performance.
Select the code that represents the reason for the temporary hiring.
Indicate if the registration is an initial contract or if it is the extension of a current one.
Select the code that represents the type of inclusion of the temporary contract.
If you wish, use this field to detail the justification of hiring.
If applicable, inform the employee's CPF that has been replaced by the temporary one.
Define the contract generation situation.
Inform the end date of the temporary collaborator's performance.
If the address of the temporary collaborator is different from the address of the borrower, enter "S - Yes" in this field and provide the address of the address in the corresponding fields (Parents, state, City, Neighborhood ,Zip code, Address , Numberand Complement).
Select an Other Company registry for the system to suggest the address information of the temporary from there.
Enter the type of company registration of the temporary. The system suggests a value for this field based on the Other Company register, when selected.
Registration Number
Non-editable field that displays the Other Company registration number, based on the code defined in the fieldType Subscription.
Statutory
Warning

In this tab, it is possible to register information of workers with contract of type statutory. It is available only when the RAIS Binding field of the binding indicated in the record of the worker’s admission binding is equal to 30, 31, 35, 95, 96 or 97.
Provision
Inform the employee of the statutory employee.
Date Appointment
Enter the date the employee was named.
This information will be taken to the eSocial layout S-2200. If this field is left blank, the employee's Admission Date will be taken to the respective fields. This field is mandatory only if the employee was admitted after the start of the Non-Periodic leasing.
Date of ownership
Enter the employee's date of ownership.
This information will be taken to the eSocial layout S-2200. If this field is left blank, the employee's Admission Date will be taken to the respective fields. This field is mandatory only if the employee was admitted after the start of the Non-Periodic leasing.
Judicial process
Enter the case number of the court decision appointing the employee. This is one of the information sent to eSocial's S-2100 layout.
When making admission of children under 16 with an eSocial category other than "103 - Employee - Apprentice" or under 14 in any category, it is necessary to inform the number of the judicial process of the permit that authorizes this hiring. This information will be taken to the S-2200 layout and will affect admissions with a date greater than the beginning of eSocial.
Apprentice Information

This documentation applies only to the module: AP.
When the employee is an apprentice and in his register the field ESocial Category is defined as "103 - Employee - Apprentice", provide here the identification data of your contracting employer.
Completion of this guide is required when the employer's affiliate is indicated as a Non-Profit Educational Entity, in which case this information is used to compose the eSocial S-2100 layout file. This guide is therefore available when:
- there is information recorded in the Insider Contractor Apprentice Information table (R034IEA); or
- the employer's employer branch is designated as Non-Profit Educational Entity in the eSocial Employer tab of the branch registration screen (FR030FIL).
Select one of the Other Companies registered in the system, to define it as the leasing contractor. The field is optional.
Enter the code corresponding to the type of registration of the learner's contracting employer. The field is required.
Enter the registration number of the learner's contracting employer. The field is required.
Buttons
Document
Through this button, it is possible to attach any image document (images up to 10MB, PDF, DOC, etc.) to the worker complementary form. When clicking on the button, a new window will be open, on which it is possible to select the desired directory and file.
Duplicate
Through this button, it is possible to reuse information from an ID number to another. For this, just select the ID number to be duplicated, click the Duplicate button, report the new ID number and confirm information through the Insert button.
